Liverpool Community Health are hosting an event on Wednesday 29th September which is aimed at patients, carers and health and social care professionals.
The event will be to promote the Liverpool Intermediate Care service, in order to raise awareness of what the service provides and an opportunity to engage with the wider community.
We would like to invite you to attend the event which will be held at Bridge Chapel Centre, Heath Road, Garston, L19 4XR from 10:00am to 3:00pm, lunch provided.
A number of market stalls will be providing an opportunity to meet with individual NHS services, charity and volunteer organisations.
The event will also launch the new patient DVD that has been produced in partnership with Toxteth TV.
